To recreate this screen capture:

Download DlgDefn.ODT
Open the document, and enable macros
Go to Tools - Macros - Organize Macros - OpenOffice.org Basic...
Go to DlgDefn.odt - Standard - DlgCode
Click Edit
Make sure you are in the DlgCode tab, and then press F5 to run the Macro (will resize the window to a standard size)
Click the DlgDef tab, and you should have the correct layout available for the screen capture.

Click on a date/time to view the file as it appeared at that time.
